Heart Attack Grill on Fremont Street is not your typical burger joint. This restaurant leans fully into its Las Vegas reputation for bold, over-the-top dining experiences, combining hearty American diner fare with a satirical hospital theme that plays with the idea of indulgence and excess. The atmosphere is playful and loud, designed for people ready to dive into both large portions and immersive themed fun.

Visitors often come hungry and leave hilariously full—many opting for the signature Double Bypass Burger, a hefty creation loaded with chilli and cheese, often paired with indulgent sides like the chilli cheese fries or jalapeño poppers. The menu also includes surprising beverage options like rich milkshakes topped with butter, or inventive cocktails that match the quirky vibe of the place.

What stands out here isn’t just the food, but the entire experience. Guests are welcomed into a tongue-in-cheek hospital setting where staff play the roles of “nurses” and “doctors”, and diners are encouraged to wear hospital gowns for the full effect. A memorable part of the visit is the playful “spanking” that happens if you can’t finish your meal — all done with good humor and clear consent, adding to the interactive atmosphere without crossing any lines.

This spot suits groups looking for a lively, slightly irreverent outing and travelers who appreciate themed venues that lean into Vegas's larger-than-life personality. The best time to visit is during the evening when the Fremont Street energy is at its peak and the retro neon signs add a colorful backdrop to the experience.

While the theme might not be for everyone, the quality of the burgers and attentive, in-character staff provide a satisfying visit. If you come with an appetite and a sense of humor, Heart Attack Grill offers more than just a meal—it’s a full-on event that fits the spirit of downtown Vegas perfectly.

My dad and I had a fun (and filling!) time at this wild spot. We totally get the message and satire behind the place, but humour and presentation aside—we actually really enjoyed our meal! It was our first stop right after landing in Vegas, and we both ordered the Double Bypass Burger (big mistake for me, should’ve gone with the Single!). We also grabbed some mozzarella sticks, which were delicious but made finishing the burger even tougher. On top of that all, for roughly $1 we got 10 strips of bacon added to our burgers! The burger itself was thick, meaty, and flavorful, with the chili on top adding a nice kick. We also loved the slushy served in a giant syringe—we mixed mango and lemonade and even took the syringe home as a fun souvenir. My dad managed to finish his meal…I, however, got too full. And if you don’t finish your plate here, the “nurses” spank you with a paddle—let’s just say, they don’t go easy on you! Overall, a really unique, entertaining, and tasty experience. Definitely a must-try if you’re in Vegas and up for something unforgettable.

This was so much fun! More than what I expected went because it was a hot spot for their food but then realized it was an experience within itself. They gave us a hospital dress to wear and were told if you don’t finish your food, you get spankings! Thankfully it’s optional but was so fun watching several wanting the experience of getting spanked

One of the funniest experiences I’ve ever had. This is one of those places you must visit if you’re traveling to Las Vegas. The burger was delicious and huge for the price. The bartenders were super funny and friendly. If you don’t finish your food, they’ll hit you in a funny way. I highly recommend coming here, but make sure you arrive hungry!

This place is amazing and a must do experience when visiting freemont street. The atmosphere alone is worth it. Some people don't get it but vegas is about having fun and that's what it is. Have been here before and will always come back. The staff is amazing and yes, you'll get spanked for not eating your food. Every man's dream. They're also very respectful about it and ask if it's okay. She even had to ask me if it was okay to spank my husband. Poor girls must've had a run in with a horribly jealous wife or girlfriend. Me? Do it girl, cause I'm gonna record it! We had a blast. It was also a heartbreaking moment because the last time I was here was with my 26yr old son who passed away in a car accident 3 years ago. We sat at the same table so it was bittersweet. My son also got spanked because he didn't finish. It's something he always talked about. Cash only in here but they also have an atm so no worries.

I thought this place was great, a cool novelty idea! The burgers were genuinely lovely with chilli con carne in them, and decent value. Loved the drinks options, had a slushie coke with whipped cream vodka. Staff were lovely too and in great character. Not really sure why people are leaving bad reviews about the concept - when the place clearly states how it works and explains before entering, so why go if you don’t like the idea. Yes you get spanked if you don’t finish your food, but also, you’re not forced into it, or maybe don’t order a ginormous meal! You can literally share a single burger if you want and avoid the whole spanking risk!! Also yes there is a scale out front but again you don’t have to use that either. It’s just a bit of fun…? Genuinely had a great time.

Cash, Cash, Cash! I really didn't expect that they didn't accept cards. To be honest, it's not very convenient. The restaurant's concept is very creative! It's a place to party, drink beer, eat huge burgers, and have fun! The burgers aren't the best, but the experience is worth it. I'm not sure if I'll come back, but it's worth checking out at least once.

I had been wanting to go here since their original location in Arizona opened in the 2000’s. Honestly I was not let down by my experience. The burgers despite was others have said, I thought were very good and satisfied my cravings. The restaurant was also a lot more relaxed than I thought it would be; plus the waitresses were very nice.

Excellent food. We got single burgers (barely finished them) I got a triple burger and it was huge. The service was great. They will try and sell you a photograph that they take so just a heads up but overall the vibes are great and the staff is well trained. They make you wear a hospital gown and treat you like a patient. If you are being bad and need a spanking, this a good place to go. I attached video for you to view (at your own discretion) And watch out the subtle details in this place. It is a very well put together burger joint with a twist.
